Covered Fertility Benefits

Your UMR plan covers fertility treatment, medications, and fertility preservation services.

In-Vitro Fertilization (IVF) coverage includes:

  • Monitoring
  • Egg retrieval
  • Intracytoplasmic sperm injection (ICSI)
  • Assisted hatching
  • Embryo transfer
  • Storage of sperm, eggs, and embryos up to the lifetime maximum

Out-of-pocket costs may include:

  • PGT-A biopsy
  • PGT-A and PGT-M testing performed by outside laboratories
  • Anesthesia during egg retrievals

Egg Freezing coverage includes:

  • Monitoring
